The fields for this part should be all self explainable. If PARTNER is "CALIBRATION", then PARTNER ="LBTB,INAF,OSURC,AZ,LBTO" is written to the script. The fields Name of PI and Porposal ID can be freely chosen, but are after every reload reset to "NOTSPECIFIED". The name of the output file is the name of the script file in the output "Script.zip", which is the standard name for the zip file, which is fixed in the moment, but can be changed later on.
